The terms research and development (R&D) and their synonyms are usually used co-terminously when referring to them in many contexts, and almost as interchangeable terms in some contexts. Terms applied in such close proximity to each other deserve a closer semantic analysis and should also attract interrogation from professional subjects involved with regard to not just their relationship to each other, but also their individual value to several contexts in regard to the quality of evidence that each offers. Laser Hair Removal: Strategies, Types and Uses

Laser Hair Removal: Strategies, Types and Uses Test patch: [10, 19] A test patch is performed by many physicians before performing the actual fist sitting of the procedure. A test patch is usually done on two or three different spots using different parameters. This helps in physician’s evaluation of the ideal treatment parameters and pain threshold for an individual case and also gives an idea to the patient about the procedure and the associated discomfort. This helps in reassuring the patient about the procedure. The patient is called after 72-96 hrs to assess the development of any delayed adverse effects and if there is no adverse sequel, the procedure can be started immediately. The energy density that is required in the first sitting is the one that caused a perifollicuar erythema and edema in the test spot.[ 30] However, a test patch may not always be feasible and is as such not recommended in all cases. It may be considered important in difficult to treat areas and in patients who are apprehensive. Laser safety It is paramount to observe safety precautions in all cases. This includes laser safety precautions and personnel safety precautions. These have been listed in box 3 Box3: Laser Safety Guidelines [19] Treatment Protocol [19,21] Pat should be asked to wash the treatment area with soap and water to remove any grease, dust or make up. Pre treatment photograph should be taken. The area to be lased should be marked with a white skin marking pencil. [Figure 38.8] Topical anaesthesia or ice cubes can be used to make the patient more comfortable during the procedure especially, if the patient is apprehensive or experienced pain during the test patch. The hair on the treatment area should be trimmed to the skin with the help of an electric trimmer or shaved with a disposable laser just prior to the procedure. The residual hair growth should not be more than 1mm -3mm. If the hair is stuck on the skin surface after shaving, these can be removed with the help of a micropore tape. The exact parameters to be used should be determined based on the test patch result or on basis of the skin and hair type of the patient. Prior to starting the procedure, it is important to recheck the parameters fed in to the system. Make a f inal check on the safety measures prior to starting the procedure as mentioned in box 3. It is especially important to confirm that all personnel in the treatment zone are wearing safety goggles. The patient should be informed before firing the laser. The hand piece should be kept perpendicular to the skin surface with firm but gentle pressure and there should be complete contact of the hand piece with skin. In case of areas such the mandibular edge, where complete contact in not possible the skin should be pulled towards the flat surface, e.g. cheek, to effect a complete contact. The pressure should be enough to reduce the capillary blood flow and push the hair follicles towards the skin surface. All areas should be treated in a single sitting. The complete area should be covered by proper placing of the handpeice with not more than 10% overlap of the treatment area. Care should be taken that the same area is not lased more than once which can lead to development of adverse effects . Care should also be taken to lase the entire treatment area and not leave behind untreated spots . This can lead to development of patchy hair growth. A treatment grid may be used to prevent these errors. Adequate cooling during the procedures can be achieved by using cool air sources or immediate application of ice packs on the treated area by an assistant. The recent equipments come equipped with cryosprays or precooled sapphire tips for intraprocedure cooling. Development of dusky discolouration or blister formation indicates incorrect parameters and these should be immediately rectified or the procedure should be stopped. Certain difficult to treat areas require special care. These are mentioned in box 4. After completing the procedure the patient should be asked to apply ice packs to reduce pain and edema. At the end of procedure a sunblock cream is applied on the treated area. The procedure details should be immediately noted and this should include the patient details and s kin and hair type, procedure details including date/ time, photograph record, fluence, pulse width and cooling method used and lastly the record of any untoward incident should be noted. Post procedure care at home should be explained and this should include avoidance of heat or hot water bags on the area for at least 24-48 hours, using mild soap and pat drying the treated area and in case if there is a blister formation or any other adverse effect, patient should be advised to apply an antibiotic ointment and inform the treating physician. Box 4: Laser hair removal in difficult to treat areas Adverse effects with Laser hair removal [30] Most complications are mild and transient such as pain erythema and edema. Sometimes superficial thermal burns may be seen. [Figure 38.9 and 38.10] These are generally caused due to wrong parameter settings, physician error during procedures, lack of adequate safety measures and inadequate sun protection by the patient. Table 6 lists the complications during a laser hair removal procedure and its management. Apart from these certain uncommon adverse effects are possible which the clinician should be aware of. Paradoxical hair growth is seen in 0.6% to 10% of patients treated as per data available from different studies. [30]. The possible etiology is the stimulation of vellus hair to form terminal hairs by the low fluences in the areas near to the treatment zones. Another theory is the synchronisation of the hair growth cycle to form terminal anagen hair from the dormant hair follicles. [ 57] Darker skin types [III-VI skin types] and patients with Hirsutism seem to be more prone to d evelop paradoxical hair growth. Continued laser treatment of the affected area is the treatment for the paradoxical hair growth. Persistent severe urticaria has been reported in a number of patients after laser epilation. Some authors believe it to be due to the delayed hypersensitivity reaction to the ruptured hair follicular antigen. [58]. Nd: YAG Laser treatment of axillary hair has been reported to cause persistent hyperhidrosis. This is possibly due to the stimulatory effect of laser on the hair follicle. [59]. Premature hair greying and ingrowing hair are some other complications reported. [30] Table 6: complications and its management during laser removal What’s New in Laser Hair removal? Newer devices are focussing on faster treatments with better patient experience. Devices with pneumatic skin flattening (PSF) help in reducing pain by working on the ‘gate theory’ of pain reduction. This theory states that on stimulation of the non-nociceptive nerves, signal transmission of pain is reduced. The device suction in the skin that has to be treated and this suction pressure on the skin stimulates the tactile nerve endings that reduces pain sensation transmission. This also helps in bringing the target chromophore closer to the laser window and reduces capillary flow which helps in optimising the treatment as has been explained above [illustration 38.4]. [30] SHRâ„ ¢ technology is another improvement that uses low fluence with rapidly delivered pulses (5-10Hz) and higher average energy. This necessitates the use of multiple passes to build the necessary energy but is faster and has a better patient experience. Another development for performing rapid treatm ents is computer controlled laser scanner mirrors. These help in automatically placing the laser beam in a perfect non-sequential pattern over a larger skin area. [60]. The technology helps in treating larger areas of various sizes by automatically addressing the issues of correct beam placement. Miscellaneous pearls in laser hair removal procedure The procedure of laser hair removal can be optimised by matching the patient and his correct parameters. Table 7 describes the ideal parameters for various skin and hair types Table 7: Laser parameters based on skin and hair characteristics As the hair structure changes from thick terminal hair to thin vellus hair, the laser parameters need to be changed to a smaller spot size. Hence a different laser may be needed as the therapy proceeds. Concurrent use of multiple laser wavelengths could have a synergistic effect and this needs to be probed further. Treatment grids are very helpful to prevent skip areas especially when a large area needs to be lased. Some patients may not respond to the therapy. In these cases, changing the wavelength may be helpful. Conclusion Laser hair removal remains one of the most commonly availed laser populations. The results post therapy are very gratifying. The learning curve with lasers for hair removal is not as steep as with some other indications. It is important not to be very aggressive, since that can lead to adverse effects. Newer technologies minimize pain and the treatment duration time. Home hair removal systems may prove to be a an essential aid in every house hold in the near future especially since the price are reducing and the newer technologies have made these equipments significantly effective. It is important to keep a standard operating protocol and following it diligently to reduce iatrogenic complications. Summary Hair has a lot of aesthetic value irrespective of age and gender. There is a decreased quality of life in hirsute patients. The laser equipments work in the wavelength range of 600-1100nm. Lasers act through the principle of selective photothermolysis and target the melanin but hair reduction occurs by the transfer of heat to the stem cells which are located in the bulge and the hair papillae. This works on the extended theory of selective photothermolysis. The parameters that have to be monitored for optimal therapy are the wavelength, fluence and the pulse width. Multiple equipments are available for hair reduction such as diode, alexandrite, Long pulse Nd YAG and IPL Home use laser hair removal devices are now available but are expensive at present. The commonest indication for hair removal is hypertrichosis in a female, however multiple other indications can be corrected by laser hair removal. Patient assessment includes detailed history and examination. An informed consent shoul d be taken prior to the procedure. The actual procedure is nearly the same with all machines with few subtle differences. Most adverse effects are iatrogenic and following a strict protocol helps in ameliorating these complications. Newer technologies are making the procedure faster and pain free.

A Comparison of Death Of A Salesman and Hamlet Willy Loman and Hamlet, two characters so alike, though different. Both are perfect examples of tragedy in literature, though for separate reasons and by distinct methods. The definition of a tragedy, in a nutshell, states that for a character to be considered tragic, he/she must be of high moral estate, fall to a level of catastrophe, induce sympathy and horror in the audience, and usually die, and in doing so, re-establish order in the society. Hamlet follows this to a "T". Death of a Salesman does not fall within these set guidelines but is still considered tragic for reasons, though different, somewhat parallel those of Hamlet's. Hamlet, a rich young price of high moral estate suddenly has his joyous life ripped away from him when his father, Hamlet Sr., suddenly passes away. Though originally thought to be of natural causes, it is later revealed to him through his father's ghost, that dear old dad was murdered by his Step-Father, and also his Uncle, Claudius. Vowing revenge upon his Uncle/Dad, Hamlet begins to mentally falter and eventually, is in such a wild rage that he accidentally kills Polonious believing him to be his father. Hilarity ensues. Ophelia, Hamlet's love interest, commits suicide/dies (that's up for debate elsewhere) after going slightly mad from the impact of her father's death, then Laertes, Polonius' son, arrives on the scene enraged and ready to kill Hamlet for what he's done, and just when you thought things couldn't get any worse, unbeknownst to Hamlet, Claudius has been plotting to kill him. Talk about your bad days. A duel takes place between Hamlet and Laertes where Laertes, using a poison-tipped sword, cuts Hamlet, thus giving way for his impending death. Hamlet eventually gets hold of the sword and kills Laertes, then kills King Claudius. Just as the play ends, Hamlet takes his last breath of air, appoints Fortinbras Jr. as the new King of Denmark, and dies. In Death of a Salesman, Willy Loman, a salesman who believed himself to be a powerful man, has his life unravel before him as he loses his job, his sanity and the respect of those around him.

God and Morality Essay

The question is this – Is it possible to be of good moral character while not believing in God? Looking at the definition of morality as defined in the Stanford Encyclopedia of Philosophy the term morality can be used descriptively or actively. A] to refer to some code of conduct put forward by a society or some other group such as religious. Or put forward by a society and is acceptable to an individual for his or her own behavior. B] to refer to a code of conduct that, given specific conditions would be put forward by all rational persons. Morality appears to be separate from religion and although not spoken of or referred to as such there are different groups of moral persons, not all are viewed as good or just. Some examples may include Christian morality, Nazi morality, Greek morality etc. At times etiquette is included as part of morality. Hobbs expresses a standard view when he talks about manners. â€Å"By manners I mean not here decency of behavior, as how one man should salute another, or how a man should wash his mouth or pick his teeth before company and such other parts of small morals, but those qualities of mankind that concern their living together in peace and unity. † (Hobbs, 2010). The laws and our legal system is distinguished by a moral system that includes having written laws or rules, penalties and officials to make the laws and enforce God and Morality pg. 3 the laws and then penalize. Laws are often evaluated on moral grounds. Moral critisism is often used to support a change in a current law or to design a new law. Religion differs from morality or having a moral system. Religious morality is based on stories or events that are past. These stories and events are used to justify the behavior that religion prohibits or requires. At times morality is regarded as the code of conduct that is put forward by religion. Morality is thought by many to need some religious explanation or justification. However, just as with law some practices and precepts are critical on moral grounds. Morals provide us with a guide for conduct, where as religion is always more than that. The definition and description of morality tells us that morality is a belief system that follows the rule of â€Å"goodness†. It is similar to a â€Å"blanket† over society and is separate from religion. Even in these times morality is always in question. In an article written by a Baltimore Sun journalist titled â€Å"The Morality Brigade. † The writer expresses his opinion onto his readers stating that â€Å"We’re still legislating and regulating private morality, while at the same time ignoring the much larger crisis of public morality in America. † (Reich, R. 2013) He continues on to back up his accusations. He writes that legislatures in North Dakota passed a bill banning abortions after 6 weeks. (At which time a heart beat is detectable) as well as approved a fall referendum that could ban all abortions by defining human life beginning at the time of conception. He continues on that these same legislatures God and Morality pg. 4 â€Å"worry about fetuses but they show little concern for what happens to children after they are born. They and other conservatives have been cutting funding for child nutrition, health care for infants and their mothers, and schools. † (Reich, R. 2013). This is just an example of American Morality. So it seems that at this point I have defined what morality is and how it can be applied. I have also touched on Religion and Morality being a very separate entity. Now I will discuss God and Morals. It is very possible for a person to not believe in God and posses high moral standards. Understanding that the person being moral is not due to their belief in God, it is independent of that. Having a belief in God is dogmatic to say the least. There is no rational way to say that God exists. To believe in God one must bypass reason and embrace faith. This is ok if believing in God gives a person comfort and security, although if we tie God to morality it may become problematic. Something as important and influential as morality should not be based on a dogma. If there is no proof that God exists beyond a shadow of a doubt than why do people still believe? Indoctrinate plays more of a role than rational and clear thinking. Religious indoctrination caters to our psychological insecurities and needs, not to reason. Think of some things that we know do not exist. An example would be the tooth fairy or maybe unicorns. We are proud to be skeptical and disapproving, however religion tells us when it comes to believing in God, faith is a virtue. God and Morality pg. 5. Morality is an autonomous human virtue that is independent of God. Otherwise, it would not be possible that atheists behave with impeccable moral standards and so many God believers do not have any morals. Let it also be noted that the great moral traditions of the world do not bring God into morality. Two of the most recognized and adored moral traditions in the east, Confucianism and Buddhism do not believe in God. The roots of morality in the west are in the idea of ancient Greeks, especially Plato and Aristotle who based their morality on reason alone, not on any dogma. It appears that religions who originate in the Middle East have a pronounced inclination to base morality on God. These same religions are also the most violent ones in history, killing many in the name of God. There is no evidence that theists are more moral than monotheists. Physiological studies have failed to find a significant correlation between the frequency of religious worship and moral conduct. An example is that convicted criminals are much more likely to be theists than not. With this being said it is a clear assumption that good people do good because they want to do good. It is not because they personally benefit from the good they do or because they are being forced. Fundamentalists perceive that the universal moral standards are required for the proper function of a society. Philosophers as diverse as Plato, Immanuel Kant, John Stuart Mill, George Edward Moore, and John Rawls have demonstrated to us that it is possible to have universal morality without having a belief in God. God and Morality pg. 6 As is stated by Paul Kurtz â€Å"One needs no theological grounds to justify these elementary principles. They are rooted in Human experience. † (Kurtz, P.1996) Now we will discuss that morality and God are linked and that religious beliefs are linked to the morality of each and every person who chooses to live their life in a moral way. If God does not exist, says Dostoyevsky’s Ivan Karamazov, â€Å"everything is permitted,† which means that we as humans can make choices whether they are good or bad, hurtful or nice. The direction in which society is going in suggests that people have lost their â€Å"way†. Too many bad things happen due to human badness. God is no longer allowed in public places, schools, government agencies, etc. This argument suggests that because God has slowly disappeared from our day to day lives society is going downhill at a very fast pace. Does this mean that God is linked to morals in our modern society? Fundamentalists claim that all ill’s in today’s society such as aids and out of wed lock pregnancy are a direct result of a breakdown in morality and this breakdown is due to a decline in the belief in God. The view that God created the moral law is often called the â€Å"Divine Command Theory of Ethics† According to this view is that what makes an action right is that God wills it to be done. All standards that we live by have consequences if not followed or adhered to. There is the fear of punishment. If we speed in our car we are given a ticket. If we God and Morality pg. 7 Sin our souls will spend eternity in hell. Heaven and Hell are often construed as the carrot and the stick that God dangles in front of to make us tow the line, to keep on the straight and narrow. Heaven is the reward and hell is the punishment. Would society be able to function correctly and morally if these rewards and punishments were not in plain view? We have lawmakers and peacekeepers all over the world to help make and enforce the laws so that people know there are consequences. Morals are no different. They help define a person’s goodness and worthiness of the ultimate reward, eternity in heaven. Moral standards without moral sanctions just simply do not mean much. Any person who thinks that it is sufficient to have relative standards that are based upon what groups or individuals feel is right or just cannot see a connection between God and morality. Moral principles such as don’t steal, don’t murder etc.must be objective and practiced by all putting aside any personal conflicts. When people accept moral principles without having a reason to they do not hold these principles as closely as someone who believes in God simply because there are no carrot or stick to hold them in place where they need to be, rather a personal belief or a mandated belief by societal rules in what is right or wrong If we recognize that God does exist than we recognize that God created the world and mankind in his own likeness. For this argument it makes sense to say that God controls the world which justifies ethical obligations. To deny God’s existence we God and Morality pg. 8 Are ethically obligated to what exactly? We typically learn morality from our parents and from those we love, we are conditioned to follow a morally righteous path in life, however to have such a standard of moral obligation can only be granted by God, binding every person and demanding such impeccable respect. By letting God into our lives we are accepting good instead of evil, we are consummating the fact that we are moral and ethical. By allowing God’s teachings and praise into our lives we are promised a more enriching morally accepted life. My view prior to doing any research was that Yes! You can lead a moral life without having a belief in God. I was raised Catholic, I even went to Catholic school, however, I have questioned my own religion over the years and now I have turned my back on it. I do bring my children to church which allows them to draw their own conclusions about God. I want them to have that choice in life and of course I would never condemn them for believing in God. It is just my personal choice. I live in the Bible belt and I always get asked the question, â€Å"where do you go to church? † I typically tell them that I do not go to church, which is similar to having the plague. It is not that I am not a good person. I feel that I am. Any morals and values that I have my parents have instilled in me. I also believe that the â€Å"rules† or commandments of God are simply a guide established at the time the bible was written to keep society in line. I absolutely agree with the fact of you do not have to believe in God to be moral. I make choices everyday and my choices are based on God and Morality pg. 9the things that I believe to be the best choice for the people I love, people I do not know and myself. I have never had any problems reaching out to help another person whether I knew them or not. I also feel that I go above and beyond at times helping others when I need to. Living a good moral life is a choice and I do it for no other reason than to be there for someone else. I do not do it for glory or self righteousness, or because someone is looking, I do it because for me it is all about treating others the way I wish to be treated. You just saw some crazy movie with Russell Crowe and that guy from the Matrix. Phaedrus: Matrix, I do not understand, what language am I speaking. Ted: Yes I know that it is strange, but due to our limited time here together I can only give you the abridged version. That was a motion picture and although I know that you were quite taken aback by it, they are not new, nor are they small in number. In fact, you would be amazed at how many have been made that have just been garbage. Anyway, I also know that Greek is a little more familiar to you but somebody more powerful than us has had their way so for the time being and so that I can understand you, we are speaking English. After that it gets really complicated so just don’t ask any more questions. Phaedrus: Fair enough I suppose, may I inquire as to why your leg is damaged as so? Ted: An injury, incurred in battle, you know war. Phaedrus: Yes, war is a term that is common in any language. Ted: No big deal really, I have learned to live accordingly, but enough about me, how about you, I hear that you are an intellectual. Phaedrus: Oh yes, more of a self proclaimed philosopher I would say. Ted: Philosophy, I see, well what did you think of the film that we just saw? Phaedrus: That was beautiful, yet it was sad, it really made no sense to me, why would Martin act as he did? Ted: Martin, what did he do? If anything could be said it would be that Andy let himself get caught up with his desires. Phaedrus: Those are foolish desires; they mean next to nothing, it was for pleasures sake only, nothing more. Ted: Oh I bet there was more, if you know what I mean, it just doesn’t happen once, not with a woman like Celia, it will only be more and more. Phaedrus: But that is not important, not at all, Martin is the loser here. Celia shall lose as well, but not as badly as Martin. Ted: Sure Martin’s losing, but Celia, I think that she did all right for herself, for she got what she wanted. Phaedrus: She did not get what she wanted. And because of that very fact Martin shall never get what she was giving. Ted: Why all the talk about Martin, he has his chance, he passed on it and Andy stepped right in, a shame really if you ask me. Martin seemed like a nice guy but didn’t come across as being that social, he should have been grateful for what he could get. Phaedrus: You perplex me with this talk of what one can get, as if the physical involvement with someone is the most important thing to have. I see no reason for your emphasis upon it. Ted: No reason, have you ever had it, or even felt it, that feeling inside, when you are in a state of total bliss, time stands still, you forget who you are, where you are and for just one moment you feel like the only two people in existence. Phaedrus: Yes, love, exactly what I am saying, know you are beginning to make sense. Ted: Love, no, no, not love, Sex, you know, sex, the act of coitus, two people throwing inhibitions aside and indulging in the ultimate act of affection. Phaedrus: No I do not understand, love is the ultimate goal, it is love to which I am referring. Love is what Celia was offering Martin. Martin felt it, perhaps he was unaware of what it was or how it felt, but he knew deep down that he was the loved one and that Celia was the lover. That she felt strong enough to do anything, even things she would not normally do in order to express her love. Martin simply pushed it away; he shunned the very thing that would bring him happiness, and virtue and all of the best in life. Ted: I just think that he pushed away any chance of getting laid, and for that I brand him a fool, it’s not everyday somebody gets an offer like that. Phaedrus: Why such a dependence upon the physical and not the emotional. I presume that you are afraid of it as well, is that right Ted? Have you ever been in love? Ted: In love, of course, tons of times, I have been in love more times than I can remember. Phaedrus: More times than you can remember, I have never heard of such audacity. No one can be in love as often as that. Were you in love, true love? Ted: What the hell does that mean, true love? I don’t know, so I slept with some women, and then I talked with them and I spent time with them, they made me happy for a while, but it never lasted long. Maybe it was just me, I don’t know. Phaedrus: Love is not that, love is the most beautiful of all the gods. To be in love, and to be the loved one, to know that you would do anything for the affection of that one person, that they would do the same for you, they would give their life to be with you or to honor the way they felt. The act in which all good things follow, that is love Ted, nothing more and certainly nothing less. That is what that film was about. Martin did not know how to handle Celia’s love, perhaps he was afraid of it, and so it removed it from his life. As for Andy, he did not receive Celia’s love, for it had been growing in her for Martin, not him. Andy was the recipient of the outlet of Celia’s frustration with Martin and his rejection of her love. Ted: That thought never crossed my mind, I just thought it odd that he turned her away when he knew that she was all over him, he didn’t have to see to know that. Phaedrus: Not lust Ted, love, and the one thing that the gods truly intended for us all. The beautiful way that two people know each other and find harmony through one another. Find it Ted, if you haven’t yet. Only that love, that feeling will make you happy. Ted: I wish it were that easy, to open up your door one day and find that someone. Yet I haven’t been looking, maybe I have never looked. Phaedrus: I am afraid that I must be on my way; I must head back on the path that brought me here. To you Ted I bid you farewell and I wish you luck and happiness and I encourage you to look beyond your ambitions to find the one thing that you lack. Ted: Goodbye and thank you for some of that philosophy. I used to only think of it as useless chatter. Ted: Yeah, a philosophy, maybe that is what I need, a philosophy. I need love. I need a wife.
